How We Approach OCD Treatment

OCD responds best to a specific combination of treatments, and getting the diagnosis right matters, since OCD is frequently mistaken for generalized anxiety or missed entirely when the compulsions are mental rather than visible.

Accurate diagnostic evaluation

We assess the specific content of obsessions and compulsions, since OCD can center on contamination, harm, symmetry, intrusive taboo thoughts, religious or moral scrupulosity, or many other themes that are not always obvious to family members.

Medication management

SSRIs are a first-line medication treatment for OCD, often at higher doses than are typically used for depression or anxiety. We manage dosing carefully and explain why that difference matters.

Coordination with ERP-trained therapists

Exposure and Response Prevention, known as ERP, is the gold-standard therapy for OCD. We coordinate closely with therapists trained specifically in ERP, since this specialized approach works best delivered by someone with dedicated training in it.

Screening for sudden-onset pediatric OCD

In rare cases, OCD symptoms in children can appear abruptly following an infection, a pattern sometimes referred to as PANDAS or PANS. We screen for this presentation when a child's symptoms appear suddenly and severely.

Family guidance

Family members often unknowingly participate in a child's compulsions through reassurance-seeking. We help families understand how to support treatment instead of unintentionally reinforcing the cycle.

Ongoing follow-up

OCD symptoms can shift in theme and intensity over time. We reassess regularly and adjust the treatment plan as needed.

In young children, I often see:

  • Repetitive rituals like handwashing, checking, or arranging objects a specific way
  • Intense distress when a ritual is interrupted or cannot be completed
  • Frequent reassurance-seeking from parents about the same worry
  • In sudden-onset cases, symptoms that appear rapidly and severely over days rather than developing gradually
Symptoms in Teenagers

In adolescents, common signs include:

  • Visible rituals, but also hidden mental compulsions like silently repeating phrases or counting
  • Excessive time spent on routines like showering, homework checking, or getting ready
  • Avoidance of specific objects, places, or situations tied to an obsession
  • Distress about intrusive thoughts they may be embarrassed or afraid to share, especially violent or taboo-themed thoughts
Symptoms for Adults

In adults, OCD often presents as:

  • Obsessions and compulsions that have been present for years, sometimes mistaken for personality traits or just being "a worrier"
  • Significant time lost each day to rituals, often an hour or more
  • Avoidance behaviors that have quietly shrunk a person's life, such as avoiding certain jobs, relationships, or responsibilities
  • Scrupulosity, involving persistent fears around morality, religion, or harming others, which patients often feel too ashamed to bring up unprompted

No. Disturbing intrusive thoughts, including violent or taboo ones, are actually a very common form of OCD, and having them does not mean a child wants to act on them or is a danger to anyone. In fact, the intense distress these thoughts cause is itself a hallmark of OCD, since the thoughts are the opposite of what the person actually wants. This is one of the most important things we can help families understand.
